To schedule an appointment, you will need to do the following:
Some insurance companies require their members to obtain a Primary Care Referral from their primary care physician, or obtain authorization from the insurance company prior to scheduling an appointment with us. Please check with your insurance provider to see if such authorization is required.
As well as your pre-authorization number, bring your insurance card, and a form of state or federal identification..
When you contact our office to schedule an appointment, our secretary will ask you a series of questions which will allow our staff to be prepared for your first visit. They will also give you any further information you will need. Additional visits may be scheduled at the time of your first appointment.
If the patient is a child under the age of 18 years, one of his/her parents will need to come to the first appointment to authorize care.
Please wear comfortable, loose fitting clothing as appropriate.
